Pulmonary and Critical Care Medicine Fellowship Program

Clinical Training

Fellows in the University of Pittsburgh Pulmonary and Critical Care Medicine program complete 18 months of structured clinical training during the first half of fellowship, gaining broad exposure across critical care, pulmonary medicine, and specialized electives:

  • 3-4 months in the Presbyterian Medical ICU
  • 2 months caring for a heme/onc population in the Shadyside Hospital Medical ICU
  • 2 months of combined Medical ICU and pulmonary consults at the VA Pittsburgh
  • 1 month of lung transplant
  • 1 month of inpatient advanced lung disease
  • 1 month in the Presbyterian Cardiothoracic ICU
  • 1 month in the bronchoscopy lab
  • 2 months of non-medical ICU time including experience in the neuro ICU, trauma ICU, among others
  • 1 month of pulmonary elective time with options including interventional pulmonary, experience in long term acute care, ultrasound, pulmonary hypertension, women’s health, global health, multidisciplinary lung cancer, additional time on physiology, consults, lung transplant, or advanced lung disease

Training includes rotations in multiple medical ICUs, with experience in general critical care, oncology, and veteran populations, as well as cardiothoracic surgery, advanced lung disease, and lung transplantation. Fellows also rotate through the bronchoscopy lab, participate in pulmonary consultation services, and complete non-medical ICU rotations in areas such as neurocritical care and trauma. A wide range of electives is available, including interventional pulmonology, pulmonary hypertension, global health, women’s health, radiology, pathology, and critical care ultrasound.

Throughout these experiences, fellows develop expertise in advanced procedures, including airway management, mechanical ventilation, bronchoscopy, hemodynamic monitoring, and pleural interventions, and assume graduated responsibility in supervising medical house staff. For those interested, additional opportunities exist in the latter half of training to serve in a junior attending role in both medical and surgical ICUs.

Training Sites

UPMC Presbyterian/Montefiore

A major quaternary referral center and home to the largest general medicine and specialty ICUs, providing fellows with exposure to complex critical care and advanced pulmonary disease. Rotations include medical ICU, consultation services, bronchoscopy, and lung transplantation.

UPMC Shadyside

A nationally recognized cancer hospital, where fellows gain unique expertise in the management of critically ill oncology patients and pulmonary complications of malignancy and cancer therapies.

VA Pittsburgh Healthcare System (VAPHS)

Provides experience caring for veterans with advanced lung disease in both inpatient and outpatient settings. The VA MICU emphasizes autonomy, procedural training, and continuity of care in a closely mentored environment.

UPMC Comprehensive Lung Center (CLC)

A state-of-the-art outpatient facility and the central site for fellowship continuity clinic. Fellows see patients across the full spectrum of lung disease and participate in both pre- and post-transplant care.

Select Specialty Hospital

Select, a long-term acute care hospital connected to UPMC Montefiore, focuses on patients with chronic respiratory failure, tracheostomy management, and ventilator dependence, often survivors of critical illness. Elective rotations here are available for second- and third-year fellows.

UPMC East (Monroeville)

UPMC East is a community hospital about 30 minutes from campus with a mixed medical-surgical ICU. Elective rotations are open to second- and third-year fellows, who function in a junior attending role.

UPMC Mercy

A level 1 trauma center where fellows rotate through the surgical ICU, acquiring experience in trauma resuscitation, postoperative critical care, and multidisciplinary collaboration.
